It seems someone is logging in at the server in the same time as you. This means your account could being hacked! Change your password RIGHT NOW! More info at http://www.happy-hg.com/forum/index.php?threads/minecraft-account-security.5988/

Still happens now and then... Changed my passwords three times, still doesn't work. Sometimes, if I hop on a server and play for half an hour and I go back. It lets me log in.

Try changing your mail and Mojang Security questions too! If someone has access to your mail it could know all the passwords changes you are doing!
