This happened just once because the server got a little problem, it wont happen again :)

i tried it and when i die i appear as spectator with the item in hand. But then when i do /ma leave to get out of the arena, the item dissapears from my hand O.o How did you do it?

I know, it seems you have a little lag in the server :( I cant fix this as this is your connection lag...

thats allowed to do and it isnt a bug, sorry :P make a mansion better hidden next time
